Not near a civil surgeon yet, or want to prepare before your appointment? Start online with Dapinder Kaur, FNP. Your virtual pre-consult walks you through the Form I-693 process, reviews your documents and vaccination records, and gets you ready — in English, Hindi, or Punjabi — so your in-person visit to seal the form is quick and stress-free.
A civil surgeon must still examine you in person and physically sign Form I-693. The virtual pre-consult prepares you; it does not replace the required exam.

FOR PHYSICIANS
Most exams follow the same four stages, wherever in Pennsylvania you book.
1
Review of your history and documents
The civil surgeon checks your ID, vaccination records and any relevant medical history before the exam begins.
2
Physical exam and TB screening
A general physical exam plus screening for tuberculosis and the other conditions USCIS requires. Blood work may be ordered.
3
Vaccinations as needed
Any required vaccinations you are missing can usually be given the same day so you meet USCIS requirements.
4
Sealed Form I-693
The surgeon completes and signs Form I-693, then seals it in an envelope for you to submit to USCIS. Do not open it.
Bringing the right documents is the difference between finishing in one visit and coming back a second time.

Prices are set by each practice. Use this as a general guide and confirm the exact fee when you book.
Exam fee
The core visit
Covers the physical exam, the review of your records and completion of Form I-693 by the civil surgeon.
Vaccines
If needed
Any required vaccines you are missing are usually billed separately from the exam fee.
Lab tests
If ordered
TB testing or blood work, when required, adds to the total cost of your visit.
Tip: ask each Pennsylvania practice for a flat, all inclusive quote so there are no surprises. Insurance usually does not cover immigration exams, so most applicants pay out of pocket.
